HCBS Survey - Your Opinion Matters

As a part of on-going efforts that will inform Georgia's Statewide Transition Plan for the Home and Community Based Services Settings Rule, the Department of Community Health (DCH) is asking stakeholders to participate in a short online survey. Submitting your responses is another way to have an impact on the future of statewide Medicaid services. Completing the survey will assist DCH to develop agreement about definitions the state will use in home and community based waiver services. DCH is looking for your responses to help answer the following questions:

  1. What are the characteristics of an isolated setting?
  2. What home- and- community- based settings are considered isolating?
  3. What supports and situations would keep a setting from being isolating?

Be sure to read the introductory information before answering the survey questions to help inform your responses.
